    The Bulletin of Legal Medicine, based on feedback from distinguished forensic science professionals, continuous to improve its quality in terms of publication process and published papers, since catching timely and regular publication in 2014. We are happy to publish actual literature reviews by highly qualified invited professionals in the field. In this issue, we are glad to share a precious article entitled “Medico-Legal Death Investigators: A Profession that could Contribute to Criminal Justice System” by our colleague Volkan Çetin from Albert Einstein College of Medicine Montefiore Medical Center, New York, USA.     Medico-Legal Death Investigators: A Profession that could Contribute to Criminal Justice System

    In the field of forensic medicine, it is essential for a forensic pathologist to gather as much information as possible, about the case in order to assign a cause and manner of death. The flow of information to the forensic pathologist is expected to be expeditious, accurate and adequate. In this article, it is aimed to introduce a profession, Medicolegal Death Investigators, who work in Medical Examiner Offices in the United States. Their duties and responsibilities in the system will be described in details. Because of their contributions to crime scene investigations, implementing a similar approach in the medico-legal investigations in Turkey will be proposed; the necessity and benefits will be discussed.     The Role and Significance of Dental Development Atlas in Age Estimation

    Atlases visualizing the development and eruption of teeth are used for age estimation in dentistry practices and in forensic sciences. This method is very practical and provides a big advantage, since it can be utilized both for living individuals and for corpses. It is especially useful and effective in identification studies during disasters with many victims. This study is a validation study of a tooth development atlas created for juvenile citizens of the Turkish Republic. Eighty panoramic teeth graphs of 5 to 22-year-old individuals with known age and sex were obtained from an imaging center within ethical principles under the condition that their personal information will be kept clandestine. The eruption and development phases of the teeth in each graph were assessed using the atlas method to estimate the age. It was found that age estimation for all graphs at and below 14 years-of-age was made with 95% accuracy with ±1 error margin and for those between 14-22 years-of-age was made with 92.5% accuracy with ±2 error margin from the data of juvenile citizens of the Turkish Republic. Obtained results suggested that the tooth development atlas created by Karadayı et al. is useful and practical for forensic age estimation and identification studies of victims of mass disasters.     Coincidental Thyroid Pathologies in Forensic Autopsies

    It's known that thyroid hyperplasia is one of the most common coincidental lesion of the thyroid gland and microcarcinomas can be seen in autopsies. In our study, it is aimed to investigate the frequencies of thyroid lesions in adult and child cases whose autopsies were performed for medicolegal assessment, in our region. The importance of autopsies was assessed to express the role of thyroid diseases as a cause or promote of death. Besides, thyroid diseases mostly seen in our region and coincidental thyroid pathologies mostly seen in autopsies were compared and discussed. In this study, it was planned to getting thyroid gland samples prospectively from cases whose autopsies were made in İnstitution of İzmir Forensic Medicine Morgue Speciality Department between April 2009 and April 2010. During the study, thyroid samples from 210 nonputrefied cases were obtained. Age, gender, causes of death, manner of death, weight and morphology of thyroid gland, thyroid pathologies were evaluated. Data were analysed using SPSS software (Version 11.0 for Windows) The cases included in the study were 161 men and 49 women. The mean age was 49.44 ± 18.25. The mean weight of thyroid gland was estimated as 40.71±27.95 gram and thyroid lesions were determined in 96 cases on histopathological examination. The prevalence of thyroid pathologies in autopsies was 45 %, the iodine intake is commonly accepted as enough in our region. There was a slight significant correlation between the gland weight and age; there wasn't any significant correlation between the gland weight and gender. The most common lesions were nodular hyperplasia (29.5%), lymphocytic thyroiditis (5.7%) and hashimoto thyroiditis (5.7%). Metastatic small cell lung carcinoma was identified in one case and congenital epithelial cyst (branchial cyst) was detected in another case. Examination of thyroid glands routinely at forensic autopsies, not only would be useful in determining the cause of death, but also will contribute to the social health data by yielding the profile of hidden thyroid diseases and especially thyroid malignancies.     Evaluation of the Sexual Assault Crimes in Muğla Forensic Medicine Department

    This research has been designed to evaluate victims of sexual assault crimes whom were sent to Forensic Medicine Department in Muğla, in last two years. Due to this purpose, 385 forensic reports issued for 341 sexual assault cases on behalf of victims between 01 June 2012 and 31 May 2014 were evaluated retrospectively. Additional to demographic parameters such as, victims' ages, sexes, relations between accused ones and victims, region of crime, and the period duration between the crime and forensic evaluation are also evaluated statistically. 90.3% of the cases were female and 9.7% of them were male. 90.6 were Turkish citizen, rest of them have different nationalities. 63.3% or the victims were younger than 18 years old, 36.7 of them were older than 19 years old. 50.1% of the victims were teen ages between 13 and 18 years old. In 23.8% of the cases accused ones were ex or ongoing boyfriend, lowers, or fiancée, in 17.3 % of the cases they were relatives. 56.3 % of the cases, it happened it house conditions. In 17.6% of the cases it has been detected that sexual assault had been going on since before 2011. 50.1 % of the cases had happened between May and August. 33.7% of the victims have been evaluated by a physician in 72 hours after the crime and 33.7 of them were performed in Forensic Medicine Department.     Carbonmonoxide Poisoning Cases Autopsied in South Marmara Region

    Carbonmonoxide (CO) related deaths, which are generally preventable accidents that include accidents due to the coal stoves and water heaters in bath at home, the mining accidents, and other accidents. CO accept as the most common cause of poisoning cases in many countries and its prominent feature is being a colorless, odorless and nonirritant gas. The autopsy records between the beginning of 2007 until the end of 2011 of Bursa Morgue Department of Council of Forensic Medicine of Turkey were reviewed. A total of 5782 autopsies had been performed and 218 (3.8%) of these involved CO poisoning. Information regarding age and sex; etiology, month and year of the accident were recorded. Study data were encoded with computer and Statistical analysis was performed using SPSS for windows program. 76 of the cases were (34.9%) female and 142 were (65.1%) male and the male/female case ratio was 1.9. The average age of the cases was 46.8 and ranged between 1 and 90 years. 57.8% of deaths were in winter markedly. The highest carboxyhemoglobin saturation was 92% in the blood. Poisoning due to CO leaks from coal heaters is an important problem in our country and surrounding regions. The mining accidents should be reduced by increasing safety in the workplace. We must more expend efforts to educate the public and prevent CO poisoning. Key words: Carbonmonoxide, Poisoning, Autopsy.

    Malpractice in Nursing: Case Reports

    The issue malpractice, which is an important and multi-dimensional concept, has been debated in recent years and as a result, an increase of penalty and compensation cases are states that will be harmful to a patient by not giving treatment, by not applying standard medical applications with intent, fault, or negligence by the health staff, and by wrong treatment applications. In malpractice cases may occur during nursing care, nurses are faced with penalty applications for being contrary to ethic principle is no damage his/her body integrity of ethical principles.     Loss of Visual Acuity due to Blank Cartridge Gun Injury: Case Report

    Blank cartridge guns are much easily accessible than conventional firearms because their appearance and sound cannot be easily distinguished from those of the real guns. Besides, they are cheap and do not require any handling license. Blank weapons; are known to be used as a tool in mutilation, homicide and suicide. In this study, we present a permanent orbital sequel in a woman who was shot in her head, by her husband using a modified blank cartridge gun, and discuss how blank cartridge guns threat public security.     Death due to Pulmonary Fat Embolism: Case Report

    Fat embolism, generally serious trauma or surgery later, is to pass to the systemic circulation of the oil droplets released from the bone marrow or the soft tissues. It is developed after long bone fractures in the young, but also after hip fracture in the elderly. The clinical table occurred usually 24-48 hours after trauma may be such as light or can progress to respiratory failure and also death. In this case, the 84-year-old woman has been admitted to the hospital due to the traffic accident with complaint pains in the left arm and the left pelvic area. On orthopedic consultation, she was made the close reduction because of the left shoulder dislocation, the left supracondylar humerus fracture and the left ramus pubic fracture, and then, she was discharged to her house. She was reported as “dead” at the house after about three hours from accident. In autopsy of forensic case regarded as suspicious death, it was seen the hemorrhagic featured liquid out on the lung sections, the left shoulder dislocation, the left supracondylar humerus fracture and the left ramus pubic deplase fracture. In the postmortem histopathological examination, it was determined the stage IV of fat embolism in the interstitial capillary lumens in the lungs and the common bone marrow embolism in the mediumsized pulmonary vascular. According to the story of the event, the contents of the medical records, and the macroscopic-histopathological examination findings during autopsy; the death of the case occurred as a result of fat embolism in the lung together with extremity fractures depending on the general body trauma after the traffic accident. After post-traumatic bone fractures, early fixation and immobilization of the fractures is important to prevent the formation of fat embolism. Being careful follow-up and treatment of the cases is important in terms of prevention of the medical practice. Also, in the cases, the broken bones in the accident identified and evaluated as suspicious death; the contribution of the autopsy and the postmortem histopathological examination is clear on determining the diagnose.     Evaluation of Methods Used in Diagnosis of Fatal Hypothermia

    Post-mortem diagnosis of hypothermia-induced deaths is still a challenging issue, today. Because of lack of findings and markers used for definitive diagnosis, cases of deaths secondary to hypothermia either remain undiagnosed or end up with an exclusion diagnosis. However, thanks to improvements in forensic sciences, literature has reported a great number of considerable methods and markers promising in the diagnosis of fatal hypothermia. In this paper we aimed to attract attention of forensic medicine professionals to the issue by discussing findings and markers suggesting and/or indicating fatal hypothermia, in the light of the literature.
